A social services organization in New York is currently seeking two APS Case Managers to join their team. These positions involve 100% fieldwork across the Bronx and Manhattan boroughs. Ideal candidates will have experience in case management and a passion for community support.

About the Opportunity:

• Hybrid or Remote: 100% Fieldwork
• Shift: Day
• Schedule: Monday-Friday
• Hours: 9:00 AM-5:00 PM, 35-hour work week
• Setting: Temp-to-Perm (6 months temp, then conversion)

Responsibilities:

• Providing direct client services, including intake and assessment.
• Managing a caseload and documenting progress in APSNET and ETO.
• Monitoring risk and adapting service plans as needed.
• Referring clients to appropriate external services.
• Conducting home visits and telephonic communication weekly.

Qualifications:

• Bachelor's degree in Social Work or related field.
• Minimum of 1 year of case management experience; 4 years preferred.
• Enrollment in MSW program is a plus.
• English proficiency; bilingual in English/Spanish is a plus.
• Strong communication, time management, and organizational skills.
• Proficiency in Microsoft Office and standard business technology.
• Physical ability to conduct fieldwork and lift/move up to 10 lbs.
