Job Title: Case Manager

Location: Denver Metro Area, CO

Salary: Starting at $22.07/hr, with potential to increase to $24.52/hr within six months based on performance.

Job Type: Full-time (40 hrs/week), Hybrid (Remote flexibility with 20–30 in-person client visits per month)

Responsibilities of a Case Manager:

  • Conduct comprehensive client assessments to determine needs and eligibility.
  • Develop and implement individualized care plans.
  • Link clients with appropriate community resources and support systems.
  • Monitor client progress and provide crisis intervention when necessary.
  • Maintain accurate, timely, and confidential client documentation using electronic records.
  • Collaborate with community partners to improve service coordination.
  • Stay informed on state and federal regulations, as well as best practices in case management.

Qualifications of a Case Manager:

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field, OR 5+ years of direct case management or social services experience.
  • Strong communication, interpersonal, and organizational skills.
  • Proficiency with Microsoft Office and case management software/electronic health records (EHR).
  • Valid driver’s license, clean driving record (MVR), and reliable transportation.
  • Must currently live in Colorado.
