Independent Contractor | Part-Time (~20 hours/week) | On-Site in Senior Care Communities

Ascend Medical is seeking a warm, experienced therapist with a passion for working with older adults to join our Behavioral Health Team. This independent contractor position is ideal for a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT) who enjoys building relationships and delivering impactful care on-site within senior living and long-term care communities.

In this role, you’ll provide in-person therapy to older adults experiencing a range of emotional and behavioral health challenges—from depression, grief, and anxiety to cognitive decline, caregiver stress, and life transitions. You’ll work closely with our interdisciplinary care team to support patients where they live, helping them feel seen, heard, and empowered.

What You’ll Do:

  • Provide in-person therapy to older adult patients in assisted living and long-term care communities.
  • Conduct psychosocial assessments and clinical diagnoses using DSM-5-TR.
  • Develop and update personalized treatment plans every 90 days or as clinically indicated.
  • Deliver evidence-based therapeutic interventions tailored to geriatric populations.
  • Address mental health challenges such as anxiety, depression, grief, adjustment disorders, cognitive decline, and caregiver fatigue.
  • Collaborate with on-site care teams, including medical providers and facility staff.
  • Session documentation, consistent with best practices and insurance reimbursement standards completed in Athena (EMR) within 24 hours of service delivery.
  • Maintain accurate, timely clinical documentation and uphold all licensing standards.

Work Environment & Compensation:

This is a 1099 independent contractor position, offering approximately 20 hours per week with flexible scheduling based on your availability and facility needs. While this role is primarily in-person, some virtual follow-ups may be arranged as appropriate.

As a contractor, this role does not include W-2 benefits such as health insurance or paid time off, but it offers substantial professional autonomy and work-life flexibility.
