What You’ll Do

As a Physical Therapist in Home Health , you’ll:

  • Complete assessments and evaluations of patient care needs related to mobility, endurance, pain, and other conditions
  • Develop individualized treatment plans and perform interventions to improve patient well-being
  • Monitor progress and adjust care plans as needed
  • Educate patients, families, and care teams on therapy goals and functional limitations

Primary Responsibilities:

  • Evaluate patient’s functional status and assist in therapy plan development
  • Deliver care within state-defined scope of practice and in coordination with the healthcare team
  • Document visits and progress notes per LHC policy
  • Instruct patients and caregivers on care plans and progress

Required Qualifications:

  • Current Physical Therapy licensure in state of practice
  • Current CPR certification
  • Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Preferred Qualifications:

  • Ability to manage multiple tasks simultaneously
  • Able to work independently
  • Good communication, writing, and organizational skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $34.23 to $61.15 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
