Job Description

Position Overview A successful Psychiatric Nurse Practitioner must be passionate about healthcare and want to make a difference in the lives of others while acting as a mission-driven catalyst to help Livingston Community Health deliver the best quality of care and excellent service to our patients and their families. The Psychiatric Nurse Practitioner will provide comprehensive primary care to the population served by LCH in accordance with the mission, vision, values and policies of the organization. The successful candidate will also provide specialized treatment and follow-up care to patients with mental health and emotional disorders. Essential Functions, Duties, and Responsibilities The Provider will deliver direct patient care.Collect, record, and maintain patient information, such as medical histories, reports, and examination results.Perform routine office examinations and procedures.Conducts evaluations and assessments to determine general physical and mental health conditions.Assess and treat adults, adolescents, and children experiencing symptoms of behavioral health disorders.Orders or executes various tests, analyses, and diagnostic assessments to provide information on the patient's condition.Interpret the results of laboratory, x-ray, and other tests.Determine and implement the appropriate course of treatment.Provide treatment plans and prescribe medications for issues/diseases.Asses and evaluates response to medication for potential side effects or adverse reactions.Counsel and educate patients on medical, psychiatric, and substance abuse conditions.Document the effectiveness of the treatment plan.Refers patients to medical specialists or other practitioner for specialized treatment as necessary.Educate patients on disease prevention and detection.Adheres to the highest standards of medical ethics at all times.Participates in clinical policy and procedural development.Attends all Staff and Continuous Quality Improvement Meetings.Observe all Livingston Community Health protocols and policies.Conform thoroughly to the current clinical protocols and Principles of Practice, abide by scheduling decisions made by the Chief Medical Officer.Document and complete each encounter in the Electronic Health Record (EHR) within 72 hours.Complete all EHR tasks including medication refills within 72 hours.Provide leadership to healthcare team through positive role modeling.Other duties as assigned by the CMO. Education, Knowledge, Skills, and Abilities Education and Experience - Graduation from an accredited Family Nurse Mental Health Practitioner school.Completion of specialized training and hold a post graduate PMHNP certificate. License/Certification - Must possess an unrestricted license (PMHNP Board Certified) to practice Medicine in California as a Psychiatric Mental Health Nurse Practitioner.Must be board certified as a PMHNP.Possess a current DEA Certificate.Current CPR Certification.Poses a valid drivers license. Knowledge, Skills, and Abilities - Excellent knowledge of the principles, practices, and techniques in the Mental Health Nurse Practitioner specialty.Excellent interpersonal skills.Broad knowledge base of physical, mental, and social needs of our underserved population.Knowledge of common safety hazards, infection control, and precautions to establish a safe work environment.Consistently demonstrates safe and accurate technical skills.Skilled in identify problems and recommending solutions.Able to effectively prepare and maintain electronic medical records, write reports, and respond to correspondence.Ability to react calmly and effectively in emergency situation.Able to establish and maintain effective working relationship with patients, medical staff, staff members, and family caregivers in a pleasant, patient, and professional manner.Computer skills required.
